Show DVD order form

One last massive well-done to you all – parents/relatives/helpers/teachers and of course – THE DANCERS for the show last weekend (doesn’t “last” sound odd!) – it was a truly wonderful thing to be part of

If you want to order a show DVD please do the following:

complete the order slip that was in the programme / attached here / on the facebook page / on the notices page of the website.  If you can’t print-off the form please write all the relevant information on a separate sheet of paper instead (“Name” refers to the dancer involved)

Indicate which show performance you would like – each DVD is the complete performance of 1 show (so there will be 3 different versions of say “Cats” on the 3 different DVD’s – you’ll have to pick your favourite show overall!)

There is a DIGITAL DOWNLOAD option available.   If you would prefer this, tick the performance choice as before and then  write  on your slip that you want a digital download and supply an e-mail address too.  Same cost as the DVD – this will be supplied instead of not as well as the DVD

Payment by cheque (preferably) to “Wizard Video Productions” – please NOT Fiona Barnard – or cash if cheque not possible.  No BACS payments please

Drop/post the completed slip together with the payment through the door of 5 High Park Road, Farnham GU9 7JJ in a named/sealed envelope

DVD’s will be supplied at the first class of term in September.

I hope everyone has a great Summer and comes back in September ready for another year of busy dancing!
